| Qualifications For Baytown Santa's Help as Follows: Any Child added to the wish list MUST ATTEND THE CHRISTMAS PARTY. Must Provide the Following: Proof of Income (paycheck stubs, SSI, food stamp, other) Proof of Expenses (lease/mortgage, utilities, loans, other) Birth Certificate for each children Parent MUST VOLUNTEER FOR ATLEAST 2 HOURS DURING THE TIME BAYTOWN SANTA SETS UP FOR DONATIONS If you know a family that needs help and they do not have access to Internet, then submit the Family's contact information to Baytown Santa. |

| If a Child is in need of a Special Item due to certain circumstances please make Baytown Santa aware of it.
Parents Please Note: You may ask for up to 4 items plus an outfit of clothes and a pair of shoes. The Value of the 4 items should not exceed $100.00 ( not including an outfit & a pair of shoes). ALL QUESTIONS ON FORM MUST BE FULLY COMPLETED. IF ANY QUESTIONS NOT ANSWERED YOU REQUEST WILL BE AUTOMATICALY REJECTED WITHOUT NOTIFICATION. |
